They would have to seriously sell all new shows at the upfronts though. Although if TR and the other new shows don't sell as well, Les has come out and said he'll hold back inventory for the scatter market (ad time purchased much closer to the time it airs).
If CBS does not put it on its fall schedule, my next guess is they would pick it up as a mid season replacement. I would think that would be expensive because they would have to keep Alex and the rest of the cast on contract for the first half of the season to make sure he was not snapped up by anybody else.
Either way, I think it would get picked up.

TR is getting very positive buzz.
CBS really wants a medical drama and TR is their production
Cons:
Not enough available spots.
But on the other hand, some of the CBS shows have gotten old and tired.
Conclusion:
TR will be picked up for the fall

Yes, Saturdays are death zone's now, but I remember staying home on Saturday nights so I could watch Dr. Quinn, Medicine Woman. How many years was that on! Friday and Saturday's were big time TV nights years ago. People wanted to see Sully and Dr. Quinn fall in love! First of all in todays climate, is networks looking past the greed of advertisers wealth, and looking at average viewers. Big high priced shows, are they in or out, can today's advertisers support them, who knows. I do know this, by fall how many cars will Cadillac try to sell during most family viewing time. Since both The Bridge and Flashpoint are being coproduced with Canadian TV (CTV), I think it would make sense to pair them together (i.e., --one is in the fall; the other one is in the spring). I also read somewhere that CBS wants to bring back comedy and to schedule new programming on Saturday nights (not just repeats) as well. So maybe that's what they are looking at doing. But who the heck knows?
